X Street Recuperative Care Facility

The Department of Community Response (DCR) will contract with an operator for the operation and oversight of a Recuperative Care Program that provides short-term residential medical respite services to individuals experiencing homelessness who have been discharged from a hospital, emergency department, behavioral health facility, skilled nursing facility, or other healthcare setting and require a safe and stable environment to recover from an acute illness, injury, surgery, or medical condition.The primary objective of the Recuperative Care Program is to improve participant health outcomes, reduce avoidable hospital readmissions and emergency department utilization, promote continuity of care, and assist participants in transitioning to permanent housing or other stable placements.The program shall operate twenty-four (24) hours per day, seven (7) days per week and provide a safe, sanitary, accessible, and recovery-oriented environment. The operator shall provide care coordination, housing navigation, benefits assistance, transportation, and supportive services designed to address medical, behavioral health, and housing-related needs.The operator shall actively pursue external funding opportunities including California Advancing and Innovating Medi-Cal (CalAIM) Community Supports, Enhanced Care Management (ECM), Medi-Cal reimbursement opportunities, managed care plan funding, hospital partnerships, and other healthcare-related funding sources. Reimbursements received through these programs shall be used to fund operations and support long-term program sustainability.
