Adult and Older Adult System of Care Services

The County of Santa Clara (County) Health System Behavioral Health Services Department (BHSD) is seeking statements of qualifications from qualified and experienced firms (Offerors), to provide a variety of services which support the Adult and Older Adult (AOA) System of Care including the Forensic, Diversion, and Reintegration (FDR) Division. Most of the programs will serve underserved adults (ages 18-59) and older adults (ages 60 and over) of Santa Clara County diagnosed with severe mental health-related problems. Programs are organized via the following categories and scopes:    Category 1 - Prevention and Early Intervention Services Scope 1: In-Home Outreach Team Scope 2: New Refugee Prevention and Early Intervention Services Scope 3: Promotores Scope 4: Elders' Storytelling Scope 5: Ethnic and Culture Specific Wellness Centers Scope 6: Older Adult In-Home Peer Respite Scope 7: Older Adult Prevention and Early Intervention Services Category 2 - Outpatient Services Scope 8: Adult and Older Adult Outpatient Continuum Services Scope 9: Adult and Older Adult Ethnic Specific Outpatient Continuum Services Scope 10: Adult and Older Adult Integrated Outpatient Continuum Services - Developmental Disabilities and Autism Scope 11: Adult and Older Adult California Work Opportunity and Responsibility to Kids (CalWORKs) Outpatient Continuum Services Scope 12: Adult and Older Adult Outpatient Continuum Services - Justice Involved Scope 13: Adult and Older Adult Intensive Outpatient Continuum Services - Justice Involved Category 3 - Full Service Partnership Services  Scope 14: Cross Systems Initiatives Full Service Partnership Scope 15: Forensic, Diversion, and Reintegration Full Service Partnership Scope 16: Assertive Community Treatment Scope 17: Forensic Assertive Community Treatment Scope 18: Assisted Outpatient Treatment Category 4 - Community Reintegration Services Scope 19: Forensic, Diversion, and Reintegration Aftercare Services Scope 20: Post-Release Community Supervision Services Category 5 - Property Management Services Scope 21: Master Lease Housing Program for Intensive Services DEADLINE TO SUBMIT WRITTEN RFSQ QUESTIONS IN PERISCOPE HOLDINGS Submit all written questions through www.periscopeholdings.com/s2g by the due date listed in Section II.A of this RFSQ.
