Minor Home Repair - Block Grant Forms

Contractors who would like to register for the Minor Home Repair (MHR) roster must first download and complete the MHR_Roster_Packet_06.21 which contains the three (3) following forms - AFFIRMATIVE ACTION FORM, LEGAL CERTIFICATIONS FORM, and the NOTICE TO LABOR UNION FORMS. Once the completed forms are received, the contractor will be eligible to receive bid notices. Please note, these forms must be completed annually, noting the Affirmative Action Form may need to be updated sooner per the instructions on the form. In addition, please note that all contractors must be trained in Prevailing Wage laws prior to contracting with the County.
