Hazardous Waste Collection for Obsolete Pesticides

All interested parties are invited to submit bids in response to this Request for Proposals. However, Bidders submitting a proposal must include the following items in order to be considered. These items will be made part of their proposal submission for evaluation along with the other proposal content items detailed in Part IV of this RFP. 1. Provide documentation of current license and certificate of insurance for hazardous waste transporting. 2. Provide proof of current HAZWOPER training for assigned staff. 3. Provide a certificate of insurance on a standard Acord form (or the equivalent) evidencing the Bidder's general liability, professional liability and any other relevant liability insurance policies that might be associated with this contract.
